While the nine qualifying group winners automatically join hosts Italy at the 2019 finals, the four runners-up with the best records against the teams first, third, fourth and fifth in their group went into the play-offs.

Greece v Austria
16/11: first leg, Salonika (17:00CET)
20/11: second leg, St Polten (19:00CET)
Poland v Portugal
16/11: first leg, Zabrze (18:00CET)
20/11: second leg, Chaves (18:00CET)
Away goals are taken into account if the aggregate scores are level, with extra time and then penalties used to separate the teams if required. The two winners will join the nine qualifying group winners and hosts Italy in the finals from 16 to 30 June 2019.

When and where is the final tournament?
The finals take place in Italy and San Marino between 16 and 30 June 2019. The venues will be:
